Free case management program to help families caring for children and adults with I/DD (intellectual and developmental disabilities.) connect to community programs, funding sources and help families navigate the network of service providers in Bexar County. The goal is to move families from a high level of dependency on case managers to greater independence and self-advocacy. Case managers connect families to medical coverage, medical providers, respite care, funding sources as well as food and utility assistance programs. Assist families in finding medical equipment, supplies and therapies. Day programs, residential services, guardianship, supported employment. Quarterly educational seminars and parent support groups. The Comprehensive School-Based Case Management (CSCM) staff works in the San Antonio community with families who have children and young adults, ages 3-21 years, with special health care needs and disabling conditions. CSCM case managers help families navigate the network of service providers and act as advocates to serve the best interests of the child. The Community Living Assistance and Support Services (CLASS) offers individuals with disabilities who need attendant care an alternative to institutionalization by helping them access the resources they need to live and work as independently as possible in the community. CLASS case managers help participants obtain personal assistance with daily activities; personal skills training; physical, occupational, speech and specialized therapies; adaptive aids; minor home modifications; and respite care.

Adult Daycare for the frail and elderly. The Elder Services Program consists of two different programs that operate within the same facility. 1) The Day Activity Health Services (DAHS)program is a state-licensed program designed to provide health services to aged and disabled persons who reside in the community. The purpose of the DAHS program is to prevent premature or unnecessarily prolonged placement in institutions. Services are designed to improve or maintain the client’s level of functioning through the provision of rehabilitative/restorative nursing and social services. Clients are then able to remain in a family environment. Eligibility Requirements for DAHS: a) Medicaid eligible outside an institution b) Have a medical condition that requires daily monitoring, such as: hypertension, diabetes, or controlled seizure disorder; c) Have a physician’s order for DAHS. Upon admission, clients must not require emergent care. DAHS clients in wheel chairs, for example, must be able to navigate on their own. clients. 2) Activity Services for the Frail Elderly (ACFE), funded by the City of San Antonio. Program seeks to meet social needs of clients 60 years of age or older in south San Antonio. Purpose is to prevent or delay institutionalization and provide respite for caregivers. The ACFE program reduces isolation and improves quality of life through social and educational opportunities, meals, active physical exercise, and preventive health services such as immunizations and screenings.
