ADRC (Alamo Service Connection), provides information, referral and assistance for both seniors and persons with disabilities. Divisions: Benefits Counseling: provides assistance to seniors 60 years of age and older and Medicare beneficiaries of any age concerning their public benefits and entitlements; Care Coordination: assesses the needs of seniors and coordinates local resources to support an individual’s highest level of functioning; Caregiver Support: assesses needs of caregivers and coordinates available support services; maintains Caregiver Resource Centers to provide literature of interest to caregivers, many of which are located in public libraries or senior centers. Nutrition: serves nourishing meals on site and/or delivered to home-bound seniors; Ombudsman: advocates for residents of long-term care facilities. Assists dialysis patients with cost of transportation to dialysis treatments using Alamo Regional Transit.

An ombudsman: Helps identify resources for residents inside or outside of long-term care facilities; Identifies, investigates, and resolves complaints by or on behalf of residents of long-term care facilities; complaints such as neglect, abuse, and exploitation of vulnerable residents. Provides information about long-term care facilities, eligibility criteria, and other services for the elderly and their family caregivers; Provides information to residents, staff, and family members about resident rights and nursing facility requirements; Advocate: works with residents, families, and staff to resolve complaints and concerns. The Ombudsman Program relies heavily on volunteers to respond to and investigate resident complaints. Ongoing training and support is provided for volunteers. Local Contact Agency Counselor (part of the Alamo Service Connection) assists Non-Medicaid nursing home residents & hospital patients transition home by providing them various options that secure their safety & well-being. The counselor will meet in person to discuss feasibility of transitioning options to provide choices, & resources to assist with possible needs. During the consumer/counselor interaction, the LCA will collaborate with vendors within the community, as well as care coordinators with the hopes of securing free or reduced cost resources(Home modifications, grab bars, home health services, Emergency Response System , adult day services, benefit counseling, transportation,& personal care supplies). Provides follow up services up to 60 days post discharge to assist the consumer with any additional needs or concerns once they are living independently.

Online support for Caregivers: free access to training and resources. unpaid primary caregivers caring for a loved one 60 years of age or older, regardless of income. ACE supports caregivers so they can maintain their health and continue to provide care so their loved one can remain at home or in the community as long as possible. Provides information and connects Caregivers with community resources. Help with caregiving tasks, such as bathing, dressing, medication management, etc. Provides connections to counseling services and support groups. Also links to other services, including but not limited to: respite, homemaker, personal assistance, minor accessibility modifications, legal assistance, emergency response systems, transportation, and home delivered meals. Many free videos. * LEGAL: If you are a grandparent or Kinship caregiver with limited resources, live in Bexar County, over the age of 55 and interested in obtaining custody, guardianship or adopting a grandchild, we may be able to assist. The Bexar Area Agency on Aging – Grandparents Program with the assistance of the San Antonio Bar Association may be able to provide grandparents/kinship caregivers with Legal assistance in order to obtain NON CONTESTED custody, guardianship or adopt your grandchild. Participating Attorneys would provide this service to qualifying grandparents at no charge.

These services can be provided at the ADRC, via telephone, or through a home visit, whichever is more convenient to the individual seeking help.

The Area Agency on Aging of the Capital Area (a division within CAPCOG: Capital Area Council of Governments) provides quality services to support and advocate for the health, safety, and well-being of the older individual in the Region.

To support the ability for consumers to make informed choices and decisions, the AAACT provides specialized services called Access and Assistance. The Older American's Act of 1965, as revised in 1973, established state Area Agencies on Aging to assist in planning and developing programs specially designed for citizens 60 years of age and older and their spouses.

The Area Agency on Aging of the Coastal Bend (AAA), a program division of Coastal Bend Council of Governments (CBCOG), housed within the CBCOG, was created by virtue of Resolution No. 234 on February 22, 1974 and is one of 28 Area Agencies on Aging in Texas. In accepting this authority, the Area Agency on Aging of the Coastal Bend assumes responsibility for the development and administration of a comprehensive and coordinated network of support services for older persons, operating under Title III of the Older American Act and Texas Department of Aging and Disability Services for Coastal Bend residents who are: 60 years of age or older Caregivers of individuals age 60 and over Medicare beneficiaries of any age. (Services limited to information and advocacy for Medicare beneficiaries under 60 years of age) Counties served: Aransas, Bee, Brooks, Duval, Jim Wells, Kennedy, Kleberg, Live Oak, Nueces, Refugio, San Patricio.

The Area Agency on Aging of East Texas is designated by the Texas Department of Aging and Disability Services to coordinate services for persons in East Texas who are 60 or older, with particular attention to low-income minority older individuals, older individuals with limited English proficiency, and older individuals residing in rural areas. Nutrition: Home-delivered meals, congregate meals, and nutritional counseling. Senior Centers: Places to go for nutrition services, employment services, and health screening, as well as a venue for social interaction and recreation. In-Home Assistance: Support for homebound elderly who want to be as independent as possible, including housekeeping, domestic chores, personal care, and visitation. Care Coordination: Arrangement and coordination of services for older people in the most efficient, economical way. Transportation: Rides to essential destinations such as nutrition sites, senior centers, doctors? appointments, and grocery shopping. Information, Referral, and Assistance: Help getting information about federal, state, or local services. Benefits Counseling/Legal Assistance: Help provided by trained benefits counselors on public and private benefits. Referrals to other sources of advice on legal matters. Nursing Home Ombudsmen: Trained and certified volunteer advocates, supervised by professionals, who visit nursing facilities and work with the residents, families, and facility employees to achieve the best possible care and quality of life. Major services include senior center meals, home-delivered meals, transportation, light housekeeping, preventive health activities, information and referral and advocacy.

HCAAA’s programs are tailored to meet the social, nutritional, educational and logistical needs of all Harris County residents age 60 and above. Nutrition services include the home-delivered meal program for the homebound elderly and the congregate meal program at senior centers throughout Harris County. HCAAA offers in-home services, caregiver training and many other supportive services.

Provides access services directly from the Heart of Texas Council of Governments (HOTCOG) offices. These services provide older persons and their family members or other caregivers efficient access to needed services, provide effective screening and assessment of individual needs, and provide advocacy for the older persons of our six county region. The Area Agency on Aging of the Heart of Texas (AAAHOT) administers Benefits Counseling, Care Coordination, Caregiver Support, Caregiver Education and Training, Caregiver Support Development and Ombudsman Programs. The Area Agency also contracts with service providers throughout the region to provide nutrition and transportation services to our older citizens.
