 Alamo Area Agency on Aging (Rural Counties)

Assists persons 60 years of age and older, especially those who are frail, living alone, low income to get the support they need to live independently in their own homes. Free services funded by the Older Americans Act and TX Dept. of Aging and Disability Services (DADS). ADRC (Alamo Service Connection), provides information, referral and assistance for both seniors and persons with disabilities. Divisions: Benefits Counseling: provides assistance to seniors 60 years of age and older and Medicare beneficiaries of any age concerning their public benefits and entitlements; Care Coordination: assesses the needs of seniors and coordinates local resources to support an individual’s highest level of functioning; Caregiver Support: assesses needs of caregivers and coordinates available support services; maintains Caregiver Resource Centers to provide literature of interest to caregivers, many of which are located in public libraries or senior centers. Nutrition: serves nourishing meals on site and/or delivered to home-bound seniors; Ombudsman: advocates for residents of long-term care facilities. Assists dialysis patients with cost of transportation to dialysis treatments using Alamo Regional Transit.

 Alamo Area Agency on Aging- Ombudsman Program (Rural Counties)

An ombudsman: Helps identify resources for residents inside or outside of long-term care facilities; Identifies, investigates, and resolves complaints by or on behalf of residents of long-term care facilities; complaints such as neglect, abuse, and exploitation of vulnerable residents. Provides information about long-term care facilities, eligibility criteria, and other services for the elderly and their family caregivers; Provides information to residents, staff, and family members about resident rights and nursing facility requirements; Advocate: works with residents, families, and staff to resolve complaints and concerns. The Ombudsman Program relies heavily on volunteers to respond to and investigate resident complaints. Ongoing training and support is provided for volunteers. Persons interested in volunteering should contact the managing local ombudsman for more information. Complaints accepted from any source, including anonymous sources.

 Alamo Service Connection (ASC) Aging & Disability Resource Center (ADRC)

Local Contact Agency Counselor (part of the Alamo Service Connection) assists Non-Medicaid nursing home residents & hospital patients transition home by providing them various options that secure their safety & well-being. The counselor will meet in person to discuss feasibility of transitioning options to provide choices, & resources to assist with possible needs. During the consumer/counselor interaction, the LCA will collaborate with vendors within the community, as well as care coordinators with the hopes of securing free or reduced cost resources(Home modifications, grab bars, home health services, Emergency Response System , adult day services, benefit counseling, transportation,& personal care supplies). Provides follow up services up to 60 days post discharge to assist the consumer with any additional needs or concerns once they are living independently.

Online support for Caregivers: free access to training and resources. unpaid primary caregivers caring for a loved one 60 years of age or older, regardless of income. ACE supports caregivers so they can maintain their health and continue to provide care so their loved one can remain at home or in the community as long as possible. Provides information and connects Caregivers with community resources. Help with caregiving tasks, such as bathing, dressing, medication management, etc. Provides connections to counseling services and support groups. Also links to other services, including but not limited to: respite, homemaker, personal assistance, minor accessibility modifications, legal assistance, emergency response systems, transportation, and home delivered meals. Many free videos. * LEGAL: If you are a grandparent or Kinship caregiver with limited resources, live in Bexar County, over the age of 55 and interested in obtaining custody, guardianship or adopting a grandchild, we may be able to assist. The Bexar Area Agency on Aging – Grandparents Program with the assistance of the San Antonio Bar Association may be able to provide grandparents/kinship caregivers with Legal assistance in order to obtain NON CONTESTED custody, guardianship or adopt your grandchild. Participating Attorneys would provide this service to qualifying grandparents at no charge.

Provides access services directly from the Heart of Texas Council of Governments (HOTCOG) offices.
