Assists families in navigating the complexities of eldercare options. AACM understands the needs of elders and their families/loved ones, providing the often neglected "management" piece of coordinating services and care for senior or persons of any age with complex needs. Care managers evaluate client situations and provide third-party, objective expertise to help families find solutions to challenging situations. Provide a single point of contact backed up by a team of professionals from the fields of nursing, social work, gerontology, government programs, financial oversight, home care and rehabilitation. Provide ongoing troubleshooting and management of client situations.

Provides services and supports to Texans with developmental disability or a related condition so that they can continue to live with their families or in their own homes. TxHmL services are intended to supplement instead of replace the services and supports a person may receive from other programs, such as the Texas Health Steps Program, or from natural supports such as his or her family, neighbors, or community organizations.

IKOR is a national firm that provides advocacy and guardianship services for seniors and those with special needs due to catastrophic injuries and/or developmental disabilities. IKOR provides Life Management services for clients of all ages and provide total person assessment (medical, pharmacy, psychosocial, environmental, educational, vocational, etc.), living arrangements, development of a personalized strategic plan, care/case management, personal rights protection, Telemedicine services, Bill Pay and Bank Reconciliation Services, POA up to Guardianship services. Ensures that clients are treated with dignity and respect as IKOR helps them navigate the complex healthcare delivery system.

Community Attendant Services (CAS) is a non-technical, non-skilled service providing in-home attendant services to people who need help with personal care tasks. CAS is available to eligible adults and children whose health problems cause them to be functionally limited in performing activities of daily living. Escort: Accompanying a person to a medical appointment. This service does not include direct transportation by an attendant. ? Home management: Help with housekeeping activities to support the person’s health and safety, including cleaning, laundry, shopping and other household tasks. - Personal care: Help with activities related to physical health, including bathing, dressing, grooming, routine hair and skin care, preparing meals, feeding, exercising, helping with self-administered medication, toileting, and transferring/ambulating. Person must be functionally limited in performing at least one personal care task, and have an unmet need for home management and personal care.
