Hospice focuses on caring, not curing and in most cases care is provided in the patient’s home. Care is covered under Medicare, Medicaid, most private insurance plans, HMOs, and other managed care organizations. A family member serves as the primary caregiver and, when appropriate, helps make decisions for the terminally ill individual. Members of the hospice staff make regular visits to assess the patient and provide additional care or other services. Services provided : Manages the patient’s pain and symptoms; assists the patient with the emotional and psychosocial and spiritual aspects of dying; provides needed drugs, medical supplies, and equipment; coaches the family on how to care for the patient; delivers special services like speech and physical therapy when needed; makes short-term inpatient care available when pain or symptoms become too difficult to manage at home, or the caregiver needs respite time; and provides bereavement care and counseling to surviving family and friends.

Provides assistance to persons with severe and chronic or terminal disability or disease who do not qualify for government assistance for whatever reason. Provides assistance for: medical co-pays, medical treatments, prescriptions, physician visits, medical equipment, medical supplies, home nursing care, home sitters, transportation, medical insurance premiums, rental assistance, utilities, deposits refundable to WeAreAlive, local phone, food, linens, clothing, personal care items, cable TV if bed ridden, microwaves if not capable of conventional cooking, travel to bring loved ones together, etc. provides assistance in directing its recipients to finding additional types of assistance that may be available for them from government agencies and other charities. Also assists recipients to find a social worker in the event they do not have one.

Application Process: Must be referred by a case manager or social worker. Will not accept applications from the general public.
