Implements a case management program to work with families on an ongoing basis to help them identify the root causes of their food insecurity and to help them develop a proactive plan to become more self-sufficient. SA Hope's model is based on providing a hand-up, rather than a hand-out. SA Hope works with a number of local nonprofit agencies to provide a variety of life skill and prevention-based programs to strengthen the family. This Program aims to educate older adults with IDD and their caregivers in health and wellness. Topics in the program will include, health, finance, spirituality, caregiver care, nutrition and physical fitness in order to strive in becoming a healthier and more efficient self-manager and caregiver.
