Comprehensive senior center co-managed by the City of San Antonio Department of Human Services and the WellMed Charitable Foundation. The center features a daily nutrition program, congregate meals, exercise stations and fitness classes, activities/recreation, and Internet cafe, WellMed health screenings, wellness education and additional services for seniors 60 years of age and older.

Offers care consultation by phone or in person to caregivers; lending library of books and videos; Quarterly Newsletter to caregivers and health professionals; Advocacy Network for rights of victims and their families; information and referral; education and training; monthly support groups. Safe Return program: MedicAlert® + Alzheimer’s Association Safe Return® is a 24-hour nationwide emergency response service for individuals with Alzheimer’s or a related dementia who wander or have a medical emergency. Provides 24-hour assistance, no matter when or where the person is reported missing. {Cost of system as of 6/13: $62.} Education: several educational programs, free of charge, such as Stress Busting Program for Caregivers.

The mission of the ARCH National Respite Network and Resource Center is to assist and promote the development of quality respite and crisis care programs in the United States; to help families locate respite and crisis care services in their communities; and to serve as a strong voice for respite in all forums. * Includes the National Respite Locator, a service to help caregivers and professionals locate respite services in their community. * National Respite Coalition, a service that advocates for preserving and promoting respite in policy and programs at the national, state, and local levels, and the Lifespan Respite Technical Assistance Center which is funded by the Administration for Community Living, Administration on Aging (AoA) in the US Department of Health and Human Services.

1) Counseling for mental health concerns, cognitive problems, substance abuse, grief and loss, domestic violence, etc. Counseling available in-office or in-home. 2) Nurturing Program: in-home parenting curriculum: 24 weeks, at no cost. Must have at least 1 child under age 3. 3) Caregiver counseling provided, paid by Bexar Area Agency on Aging.

Online support for Caregivers: free access to training and resources. unpaid primary caregivers caring for a loved one 60 years of age or older, regardless of income. ACE supports caregivers so they can maintain their health and continue to provide care so their loved one can remain at home or in the community as long as possible. Provides information and connects Caregivers with community resources. Help with caregiving tasks, such as bathing, dressing, medication management, etc. Provides connections to counseling services and support groups. Also links to other services, including but not limited to: respite, homemaker, personal assistance, minor accessibility modifications, legal assistance, emergency response systems, transportation, and home delivered meals. Many free videos. * LEGAL: If you are a grandparent or Kinship caregiver with limited resources, live in Bexar County, over the age of 55 and interested in obtaining custody, guardianship or adopting a grandchild, we may be able to assist. The Bexar Area Agency on Aging – Grandparents Program with the assistance of the San Antonio Bar Association may be able to provide grandparents/kinship caregivers with Legal assistance in order to obtain NON CONTESTED custody, guardianship or adopt your grandchild. Participating Attorneys would provide this service to qualifying grandparents at no charge.

Resource centers that offers complimentary programs designed to support family caregivers in their caregiving role, including one-on-one assistance, education, wellness activities and more. Centers commonly serve caregivers with loved ones impacted by Alzheimer’s disease and other chronic conditions such as stroke, cancer, diabetes, chronic lung disease and heart disease. Caregiver advocates staff each center to provide education, emotional support and training.

The Caregiver Teleconnection is a free, bilingual, and confidential program that connects caregivers and family members to information and support through the telephone or tele-learning. The Caregiver Teleconnection provides learning sessions on a variety of useful topics, hosted over the phone by professional facilitators and experts. Family caregivers have the opportunity to connect and share with others in their situation during the sessions. Makes learning and support simple: just a phone call away. Call. Learn. Share. If you’ve ever used a telephone, you have all the tools and skills you need to participate in tele-learning, which simply means learning over the phone. Each tele-learning session is intended to empower you to make positive life-changing choices for you and the person for whom you care. The sessions can accommodate a large number of caregivers. Our trained, professional facilitators make sure that each session flows smoothly, and invite participation of the caregivers on the phone. Some of the topics included in the tele-learning sessions are: Caregiving 101, Top 5 Ways to Thrive as a Family Caregiver, The Tell Tale Signs of Caregiver Stress, Living Guilt Free, For Better or For Worse: The Challenges of Being a Spousal Caregiver. Participation in the Caregiver Tele-Connection helps prevent caregiver burnout, depression and anxiety, social isolation and other problems that threaten your health. The program can help you gain a greater sense of control and improve your quality of life. Focusing on caregivers of persons with Alzheimer’s or other dementias, Caring for the Caregiver is dedicated to improving the quality of life for family caregivers and those they care for with evidence-based services, support, education, and research with value on the caregiver’s health, skills, and resilience. This program is offered through the School of Nursing at UT Health San Antonio.

Caregivers can search for resources by state. Provide caregivers with online training & education that improves the quality of care received by older adults. mmLearn.org provides free online video webcasts that are easily accessible on a computer or mobile device & include practical skill training (how to transfer someone), dementia training, elder nutrition, medications, depression, hearing/vision, family dynamics & caregiver tips & other crucial issues that caregivers face everyday.

National Stroke Association's mission is to reduce the incidence & impact of stroke by developing compelling education & programs focused on prevention, treatment, rehabilitation & support for all impacted by stroke. Website has a support group registry, where one can search for a group nearest to home. A large section of the website is devoted to CAREGIVERS, with much valuable information, including downloadable books, support groups, webinars and videos.

Care Management is our approach to addressing the entire well-being, comfort & functionability of our clients & their families. Our care managers are registered nurses (RN) & social workers (SW). They provide every client a personalized care plan that is developed from a collaborative assessment with the client & family. SeniorBridge Care Managers deliver services through a variety of activities, approaches & solutions. A client with SeniorBridge can receive a range of customized, high quality care that allows them to remain as independent as possible and safely at home. The SeniorBridge services include: Collaborative assessment of the client's needs. Developing a comprehensive plan for care of the client. Creating a customized approach to care where a person can receive the appropriate level of care for their specific needs. Coordination of all services and care provided to clients, including doctor and other professional appointments and social activities. Train, orient, supervise and support in-home caregivers and other staff. Provide direct, in-home care from Registered Nurses, Social Workers and Home Health Aides. Establishing home safety and emergency preparedness. Attention to psychological factors, such as family, friends, daily routine, activities and family caregiver stress. Assessing financial and legal factors that are relevant to the client??s ability to plan and pay for long-term care needs.

This Program aims to educate older adults with IDD and their caregivers in health and wellness. Topics in the program will include, health, finance, spirituality, caregiver care, nutrition and physical fitness in order to strive in becoming a healthier and more efficient self-manager and caregiver. Sessions are 2 to 3 hours for 6 weeks (Series of 6 classes once a week)

Relatives and other people who the child or family have a significant relationship can often provide children with stability when they can't live with their birth parents.
