Prescription Assistance Program (PAP) enables families without medical insurance or who have exhausted all other insurance resources to apply for free or low cost prescription drugs. If you meet one of more of these guidelines you may qualify for our program: You have no health insurance or limited benefit coverage; You have exhausted your coverage on Medicaid; You have exhausted your coverage on Medicare Part D; You are pending CHIP or Medicaid; You have a chronic condition and need to receive medications regularly; You meet the income guidelines set by the pharmaceutical company; You have a regular physician. (The McKenna Site for Prescription Assistance is located in the New Braunfels office.)

Federally Qualified Health Center (FQHC),non-profit, provides services to all. Provides accessible, comprehensive, quality care services to all residents of Atascosa, Karnes and Wilson Counties. AHC achieves this mission with a network of four rural health clinics. Includes dental and behavioral health services. Family medicine, acute care, chronic disease management (diabetes, etc.), women’s care, pediatrics, senior care, dental care, behavioral health, mental health and family counseling.

Offers limited financial assistance to those who qualify. Services include utility assistance, prescription assistance, and information and referral.

CIHCP (County Indigent Health Care Program) provides medically necessary health care benefits to individuals who meet the income, resource, residency and household criteria; and who are ineligible for other state or federal assistance programs. This is not an insurance program. CIHCP pays for basic, medically necessary health care services such as: Physician services Annual physical exams Immunizations Medical screening Laboratory and X-Ray services (ordered by physician) Family planning services Skilled Nursing facility (ordered by physician) Prescriptions (max 3 per month; ordered by physician; no over-the-counter) Rural health clinic services Inpatient and Outpatient hospital services Also: Advanced Practice Nurse services Ambulatory Surgical Center services Diabetic Medical supplies and services Physician Medical supplies and services Income Requirements: Generally a person with earned income could earn up to $389 per month and still meet the income limit of 21% of the Federal Poverty Income Level (FPL) for a household of one. Those with unearned income have a different calculation applied.

Easy, friendly helpful access to services for older adults, people with disabilities and their families in the 12 county Alamo area. Information, referral, and assistance system for seniors, people with disabilities, and their families. Provides assistance in navigating the complex system of services offered by government agencies, nonprofits, and other service providers. ASC has an extensive database on its website, which is supported by systems navigators in the ASC call center. Systems navigators help callers explore options for handling problems related to aging and disability and recommend resources and strategies for problem resolution. The database is also accessible to the public. Go to www.askasc.org for a resource directory for seniors, caregivers, and persons with disabilities.

One-time emergency financial assistance to prevent utility disconnection, financial coaching, and budget assessment for income-eligible individuals and families. Case management staff provides a thorough intake and assessment of the family’s financial state in order to provide the best possible plan to help them move toward financial stability and self-sufficiency. Also utilize the Clothes Closet and Food Pantry to assist clients with obtaining their basic needs at no cost, reducing the client’s cost of living expenses. Referrals for additional Catholic Charities programs, employment, community resources and government assistance will also be provided to those in need. If eligible for financial assistance through this program, Catholic Charities will remit payment directly to the vendor.

Good Days is a non-profit advocacy organization that provides resources for life-saving and life-extending treatments to people in need of access to care. Good Days offers patients and families financial help, emotional support, and guidance to the best available information and support for each specific disease state. Since 2003, Good Days has provided more than 800,000 grants and helped more than 500,000 people with access to healthcare resources. CommuniCare Health Centers is a full-service primary healthcare system offering pediatric medicine, family medicine, senior care, women’s health, podiatry, dental, behavioral health, after-hour urgent care, optometry and optical services, minor surgery clinic and fitness and wellness services in Bexar and Hays counties. * Five Clinics in San Antonio (each has a separate listing): West Campus (Barrio Family Health Center), East Campus, Northwest Campus, Downtown Campus, and Potranco Campus. * There is also a WIC (Women, Infants, and Children) Center at 803 Castroville Rd, Ste 416, SATX 78237. ** HEALTH INSURANCE MARKETPLACE: CommuniCare Health Centers has been designated by the Federal government as a Certified Agency to provide application assistance to those people seeking insurance coverage through the Health Exchange. CommuniCare has created an Insurance Enrollment Department staffed by trained and "Certified Application Counselors" who can assist people through the entire enrollment process, in English or Spanish, beginning October 1, 2013. Prior to October 1, Counselors are available to answer questions and provide information about how to prepare for enrollment. CommuniCare Certified Application Counselors are able to assist with enrollment in the Exchange, Medicaid, CHIP, and Medicare plans. You can contact CommuniCare’s Insurance Enrollment Department at (210) 233-7157 to speak with a Counselor.

Services at this location include: pediatric, family medicine, podiatry, women’s health, pediatric and family dentistry, behavioral health, and optometry and optical services. Provide comprehensive and preventive medical care (pediatrics, family medicine, internal medicine). Services include obstetrical care, immunizations, annual physical exams, and family planning. Also pediatric acute and well-child care are offered. Adult acute and chronic disease (diabetes/heart disease) management and podiatry services are available. Health Education services include individual and group counseling for chronic disease management, nutrition, weight loss, and exercise. Pharmacy available on site. On-site screening and application services for Medicaid and other public insurance programs.

Assists families who have a sudden or unexpected occurrence with financial assistance limited to electric, water, propane, pharmacy or rent.

Lunches free of charge, health and nutrition education. Case management including assistance with prescriptions, medical equipment, employment referrals. Medical and dental screening by WellMed Medical Management. Also fitness activities, computer classes, and many recreational activities. Free transportation Mon-Fri within 5-mile radius of center. Also, 3 VIA bus lines are within walking distance, and there is a covered bus stop in front of the building.

Offers crisis intervention and assistance with personal and family problems requiring profession direction. Provides emergency assistance with food, clothing and medication, and emergency utility assistance for those in specified areas of town (Southside; see zip code list. Also serving Somerset, Jourdanton, and Von Ormy.) A food distribution center for the San Antonio Food Bank’s Project H.O.P.E commodity program. Distributes free groceries to persons aged 60 and older who meet financial eligibility criteria.

Many programs to provide help for families that have had an unforeseen emergency. Other programs offer support that makes families more stable and self-sufficient. 1) Rent assistance, mortgage or utility payments. This is one time help that enables the family to survive a crisis. 2) Clothes Closet for families in the New Braunfels area in need of good used clothing and household goods. Service is by appointment and the clothes are free. Call for an appointment at 830-625-7100. 3) Food Pantry (by appointment) Donations and volunteers welcome.

Assist with rent, utilities, fuel, and short term prescription assistance. Also can pay for GED testing.

FamilyWize acts like a big buying group, pulling together millions of people that either have no health insurance (the uninsured), have high medication costs, or need to buy medicine not covered by their plan (the underinsured). If desired, you may register online, but is not necessary to receive discounts. You can also print out your free card.

Services provided include physician visits, hospital inpatient and outpatient services, up to 3 prescription medications per month, and may include payment of existing medical claims. This program is also available to undocumented persons who meet eligibility requirements. Indigent Health Care is, by law, the payer of last resort, and all other possibilities will be explored first (when medically appropriate to do so). Households with minor children need to be screened first for Medicaid eligibility.

Provides health care for Gillespie County citizens who meet program requirements (place of residence, income, resources). Services provided include physician visits, hospital inpatient and outpatient services, up to 3 prescription medications per month, and may include payment of existing medical claims. This program is also available to undocumented persons who meet eligibility requirements. Indigent Health Care is, by law, the payer of last resort; all other possibilities will be explored first (when medically appropriate to do so). Households with minor children need to be screened first for Medicaid eligibility.

Charitable medical and dental clinic that helps low-income, uninsured individuals and families who live below 200% of the FPL. Do not see individuals with Medicare, Medicaid, or CHIP. However, if your insurance does not include dental insurance, you may be eligible for dental clinic. Examples of types of services: preventative care, infections, injuries, recent onset of pain (head, stomach); Diabetes management, high bp, asthma, uncomplicated dental extractions, fillings, and cleanings. Guadalupe Valley Hospital provides health care for Guadalupe County citizens who meet program requirements (place of residence, income, resources). Services provided include Guadalupe Regional Medical Center, inpatient and outpatient services, and may include payment of existing medical claims. This program is also available to undocumented persons who meet eligibility requirements. In addition to providing support during an illness or crisis, chaplains are trained to offer assistance with: * Advanced Care Planning * Living Wills – Written instructions that explain your wishes regarding healthcare should you have a terminal or irreversible condition. They are called living wills because they take effect while a patient is still alive. * Medical Powers of Attorney (MPOA) – In a written document, you can name a person to be your healthcare agent. This agent will make decisions for you if you become unable to do so. * Out of Hospital DNR (Do Not Resuscitate) An Out of Hospital DNR is NOT an Advance Directive. It is an actual order from your physician. It directs that you not be resuscitated out of the hospital regardless of the condition of your health. These orders are usually reserved for individuals who are terminally or irreversibly ill and possibly receiving Hospice Care. This form when presented to any Emergency Medical Personnel will stop them from providing any treatment (i.e. CPR, defibrillation, artificial ventilation) that would restore any cardiac or respiratory function. * Bereavement or Grief Support * Organ and tissue donation

The Prescription Assistance Program (PAP) at GRMC helps patients receive needed prescription medications to treat chronic medical conditions such as asthma, diabetes, high blood pressure, high cholesterol, and depression. Qualification guidelines for PAP are: US Citizen, Have a low/fixed income, Are on long-term medications, A physician in Guadalupe County Can pay $15/month. Offer emergency assistance with rent, utility, counseling, domestic violence issues, etc. Limited financial assistance for emergency dental care. Other services include: * Help a Senior (HAS): Volunteers provide one on one "friendly support to a senior living alone with no family to look out for them. * Navigator: a volunteer who assists in managing personal and financial paperwork and correspondence. * Yard Care and Handy Man Services: for elderly and disabled individuals. * Angel Tree and Christmas Families: community members adopt an angel representing a lonely senior or disabled home-bound adult, providing gifts and Christmas dinner. * School Supplies: for children of working poor families.* Warm Winter/Cool Summer: coasts, blankets, heaters and fans for those in need.

Hill Country Memorial Community Services is designed to meet an extensive range of community health care needs including Licensed nursing, home health aids, occupational, physical, and speech therapies, homemaker services, and medical social worker. Can also provide financial assistance with medications to qualifying seniors residing in Gillespie County only.

Services provided include physician visits, hospital inpatient and outpatient services, up to 3 prescription medications per month, and may include payment of existing medical claims. This program is also available to undocumented persons who meet eligibility requirements. Indigent Health Care is, by law, the payer of last resort, and all other possibilities will be explored first (when medically appropriate to do so). Households with minor children need to be screened first for Medicaid eligibility.

Pays for basic health care services such as annual physical examines, laboratory, x-rays, & inpatient/outpatient hospital services. Assists with optional health care services such as home and community health care services, diabetic & colostomy supplies & equipment.

LI NET provides immediate point-of-sale prescription drug coverage for people with LIS (Extra Help) who are NOT YET ENROLLED in a Part D plan. Beneficiaries may be qualified for a Retroactive Reimbursement coverage for months that they were not on a Part D plan and retroactive coverage & paid out of pocket. All enrollees are temporarily covered until they are enrolled in a Part D plan.

County Indigent Healthcare Program is a county program that provides medically necessary health care benefits to individuals who meet the income, resource, residency, and household criteria; and who are categorically ineligible for the Texas Medicaid Program.

Offers board certified family practice physicians, pediatrics, adult medicine, women’s health & wellness, laboratory, immunizations, gynecology, electrocardiograms, & physical exams to persons who are doing without medical care due to the expense of health insurance. * Also has counseling center, offering case management services to include information & referral, referrals for training & employment assistance, assistance with applications for CareLink, TexCare, Medicaid & Medicare. Also offers individual & family counseling.

Provides a comprehensive searchable list of prescription assistance programs. NeedyMeds Drug Discount Card saves you up to 80% or more off the cost of: prescription medicines, over-the-counter drugs, and pet prescription drugs.

Web site and call center have extensive database of discount or free prescription medications available according to eligibility requirements. Partnership Prescription Assistance (PPA) determines which program to refer to based on inquirer responses to questions about eligibility, trying to get the least costly alternative. Program takes approximately two weeks from the time the caller completes and returns a form sent to them in the mail or downloaded over the Internet.

A Medicare-certified charity which provides direct financial support to insured patients, including Medicare Part D beneficiaries, who must financially and medically qualify to access pharmaceutical co-payment assistance. The program offers personal service to all patients through the use of call counselors; personally guiding patients through the enrollment process. Counselors can assist with speaking to your insurer, employer and/or creditor regarding matters of insurance, job retention and/or debt. If you are suffering from certain types of cancer or macular degeneration, CPR may help pay your Medicare private drug plan out-of-pocket costs and what CPR pays will count toward your $3,600 out-of-pocket costs. We currently assist patients diagnosed with: Breast Cancer, Colon Cancer, Lung Cancer, Lymphoma, Kidney Cancer, Prostate Cancer, Sarcoma, Macular Degeneration, Secondary issues as a result of cancer treatment.

Many pharmaceutical manufacturers also offer a limited supply of free or low-cost medications to patients in need through their Patient Assistance Programs (PAP’s.) These programs are intended to serve as a "last resort" for patients who are unable to use other programs and who otherwise would not be able to afford needed medications. RX Assist is a searchable database that allows you to search for PAPs by company, brand name, generic name and drug class. The information includes eligibility and application instructions. The application must be submitted by your physician. Comprehensive directory of Patient Assistance Programs. Patient assistance programs are run by pharmaceutical companies to provide free medications to people who cannot afford to buy their medicine. RxAssist offers a comprehensive database of these patient assistance programs, as well as practical tools, news, and articles so that health care professionals and patients can find the information they need.

Medicare recipients with limited income & resources may qualify for Extra Help to pay for the costs -- monthly premiums, annual deductibles & copayments. Monthly income limit is $1459 for an individual and $1966 for a couple. Resource limit is $13,300 for individual & $26,580 for a couple. Provides financial assistance to eligible cancer patients.

The CareLink Program is open to San Antonio/Bexar County residents who do not have private or public health insurance. Based on several factors, like the number of people in your family and your total income, you may qualify to be a member of CareLink. Once enrolled, CareLink will establish a monthly payment plan based on total household income and family size. You will be assigned your own primary care physician and have access to quality healthcare at University Health System.

The purpose of the VFW is to speed rehabilitation of the nation’s disabled and needy veterans, assist veterans’ widows and orphans and the dependents of needy or disabled veterans, and promote Americanism by means of education in patriotism and by constructive service to local communities.

Provides assistance to persons with severe and chronic or terminal disability or disease who do not qualify for government assistance for whatever reason. Provides assistance for: medical co-pays, medical treatments, prescriptions, physician visits, medical equipment, medical supplies, home nursing care, home sitters, transportation, medical insurance premiums, rental assistance, utilities, deposits refundable to WeAreAlive, local phone, food, linens, clothing, personal care items, cable TV if bed ridden, microwaves if not capable of conventional cooking, travel to bring loved ones together, etc. provides assistance in directing its recipients to finding additional types of assistance that may be available for them from government agencies and other charities. Also assists recipients to find a social worker in the event they do not have one.

The clinic provides services that include primary medical care, specialty referral, counseling, health education, nutrition counseling, WIC, and transportation assistance.
