 Methodist Healthcare Ministries (MHM) / Corporate Office

1) Clinical Services. MHM owns & operates two primary care clinics at 2 locations in San Antonio. The clinics address medical & dental needs of people who cannot afford health insurance. Services are provided on a sliding scale fee based upon ability to pay; household income & family size are factors. Case management & social services are also provided free of charge. Operated by licensed & professional counselors. The counselors meet with individuals privately at a local church. 4) Nutrition & Health Education: health education program is provided to clinic patients on an individual & group basis. Many issues, such as diabetes, weight management, asthma & heart health. Health education services are available at the Bishop Ernest T. Dixon Jr. Clinic & the Wesley Health & Wellness Center. 5) Wesley Nurse Health Ministries: (WNHM) a faith-based, holistic program committed to serving the least served through education & health promotion. 6) Parenting Programs: practical support to help parents & guardians learn parenting skills. Meld is a parent support program for share parenting information & learning from other families facing similar challenges. Meld Plus is a home visiting program that promotes optional child development & positive parent-child relationships.

 SA Hope Center

Implements a case management program to work with families on an ongoing basis to help them identify the root causes of their food insecurity and to help them develop a proactive plan to become more self-sufficient. SA Hope's model is based on providing a hand-up, rather than a hand-out. SA Hope works with a number of local nonprofit agencies to provide a variety of life skill and prevention-based programs to strengthen the family. Emergency Food and Clothing, Case Management Monday thru Friday 9 am - 5 pm and Saturday 9 am - 2 pm.

(SAFB) provides food and grocery products to more than 500 partner agencies in 16 counties throughout Southwest Texas. SAFB offers assistance with applying for public benefits and also offers DaisyCares, a program that assists struggling families & individuals with pet food assistance for their pets.  Support and Empowerment for Older Adults with IDD and their Caregivers

This Program aims to educate older adults with IDD and their caregivers in health and wellness. Topics in the program will include, health, finance, spirituality, caregiver care, nutrition and physical fitness in order to strive in becoming a healthier and more efficient self-manager and caregiver. Sessions are 2 to 3 hours for 6 weeks (Series of 6 classes once a week)

Target Group: Individuals diagnosed with IDD or caregivers who care for someone with IDD

Eligibility Requirements: Must have IDD or be caring for someone with IDD / caregiver does not have to live with individual with IDD and caregiver can be paid or unpaid.
